Daily Screening Checklist
Please answer the following questions for yourself as a staff or visitor or on behalf of your child each morning prior to attending Munchkin Place Home Daycare, FUN Kid's Yoga or Munchkin Place Art classes. Please do not attend school or classes while ill. Please notify Munchkin Place if your child or household members have symptoms of COVID-19 and or have tested positive for the Covid-19 virus.
Below is the link to the Ontario School and Child Care Online Screening Assessment. Please note, Munchkin Place Home Daycare is located in Durham Region and will abide by Durham Region Public Health Guidelines.
